The "clicking all over the place" is part of the problem I also have, but this is consistent with the pointer jumping to the lower left of the screen. Checking for loose connections has also been tried. The scroll wheel provided with the NYLXS boxes was one of the mice that exhibits this problem with SUSE 7.3 and if I remember correctly, the 7.2 release of Red Hat. The problem is not limited to scroll wheels, but also with a regular center button mouse, and various two button mice. Also, again if I remember correctly, the problem also showed up with a serial plug mouse.
As for the kvm switch, I found a mouse that works, and am using a separate mouse for each box, with the kvm switch handling the keyboard and monitor duties. Also, the kvm switch, although a separate problem than the problem specified above, has its own problems. I did a lot of searching in the groups on this issue about 4 months ago. There is a problem (although getting some people to admit it is difficult) with the Linux kernel, in regards to the system losing the signal from the mouse when you switch between boxes. This was initially blamed on various kvm switches, and their manufacturers, but was subsequently blamed on the kernels, as each manufacturer can't be responsible for the same problem, and the problem can't be reproduced with M$/Apple/other boxes. Do a search on KVM, mice, and Linux and you'll come up with the postings.
As for selecting correct mouse, if it wasn't on list, I generally selected generic 2/3 button ps2 mouse, or whatever seemed to make the most sense. In going back and trying to solve the problem, one thing I may not have done was reboot each time when selecting new settings, but reboots have happened subsequent to that, and I still have the problem. I'll be trying additional mice when I get a chance, and will make sure to reboot after changing settings each time.
The problem has shown up both in AMD motherboards, and Intel motherboards. I'm putting together another system now, and I'll see if the problem persists with the same mice that I'm having problems with now. If I can reproduce the problem with this new system, running SUSE 7.3 with stock kernel, I will be updating the kernel to 2.4.18, and I'll let you know if the problem persists with the new kernel, or if that solves the problem.

it may sound silly, and i'd figure this was an early tried remedy but i might as well throw it out there... i had the same exact problem with MDK 8.2, MS PS/2 Intellemouse w/ wheel, and a Belkin 4 port KVM switch. It was a loose connection between the KVM cable and the ps/2 port on the linbox. Again very basic but someone can always lose their glasses on the top of their head. I have also experienced the mouse jumping problem with my KVM switch. The mouse works great until I switch the KVM switch and then sometimes it continues to work and other times it completely freaks out. I end up using SSH from Windoze to restart the linux box so that the mouse will work again.
Also, my cursor doesn't just jump all over the screen, it also spuriously opens menus and basically "clicks" all over the place.
I never had a problem before using the KVM switch.
As a final note, the mouse has never exhibited such bizarre behavior on my Windows 2000 machine which is connected to the same KVM switch.
Of course,if somebody has a fix for this it would be greatly appreciated, because I hate using Windows 2000 as a "restart" for my Linux box. It just doesn't feel right. :(
